All of the clinicopathologicparameters before and after neoadjuvant chemotheraphy of 96 female patients with localbreast carcinoma treated by neoadjuvant chemotheraphy between years of 2005 and 2013were analyzed retrospectively. Pathologic stages were found as no residual tumor in 11patients (%11,8), microscopic tumor foci in 21 patients (%22,6). Overall survival rate of allpatients was obtained as 75%. Among all oft the clinicopathologic parameters, onlyrecurrence was found to be statistically related to survival (p=0.001). However, therelationship between survival of patients withcomplete pathologic response of breast only,axilla only and breast-axilla together was found to be higher, this relationship was notstatistically significant. Survival rate of the patients with complete pathologic response wasfound as %68,8. Pre- chemotheraphy advanced stage, nipple involvement and multicentrisitywere found to be inversely affecting the disease free survival (p=0.033; 0.009; <0.0001respectively). Accordig to the subtypes of breast cancer; the shortest disease free survivalwas found in hormone receptor(-) HER2(+) and hormone receptor(-) HER2 (-) patients withmedian survey of 13,9 months and 16,2 months whilst the longest disease free survival wasdetermined in hormone receptor(+) HER2(-) cases (median survey 59,8 months). Localrecurrence free survival rate was calculated as %92. Response to chemotheraphy and estrogenpositivity was found to inversely affect on local recurrence free survival (p=0.026; 0.020respectively).In our study; recurrence, pre-chemotheraphy stage, response to chemotheraphy, nippleinvolvement, multicentrisity, and estrogen receptor positivity are established as effective onoverall and disease free survival. Other parameters such as young age (<40), tumor size,axillary involvement, chemotheraphy type, lymphovascular invasion and progesteron receptorpositivity are found to be ineffective on survival.

Bu çalışmada LİMK hastalarındaneoadjuvan kemoterapiye yanıt ve hastaların genel - hastalıksız sağkalımı üzerindeklinikopatolojik faktörlerin etkisi araştırıldı. 2005 - 2013 yılları arasında LİMK tanısylaneoadjuvan kemoterapi almış 96 kadın hasta retrospektif olarak değerlendirildi. Hastaların KTöncesi ve sonrası , klinikopatolojik özellikleri incelendi. Hastaların NAK sonrası patolojikevre dağılımlarında %11,8'inde (n=11) rezidü tümör dokusunun olmadığı, %22,6 'sında(n=21) ise tümörün mikroskobik odaklar halinde olduğu gözlendi. 96 olgunun genel sağkalımoranı istatistiki olarak %75 olarak saptandı. NAKT sonrasında değerlendirilenparametrelerden sadece nüks varlığı genel sağkalım üzerine olumsuz etki eden bağımsız riskfaktörü olarak bulundu (p=0,001) . Meme, aksilla ve hem meme hemde aksillada patolojiktam yanıtı olan hastaların genel sağkalım sürelerinde uzama olsa da istatistiksel olarak anlamlıdeğildi. Hastalıksız sağkalım süreleri ise %68,8 olarak saptandı. KT öncesi ileri evrenin(p=0,033) , opere olabilen ancak meme başı tutulumu olan hastaların (p=0,009 ) , multisentriktümör odağı varlığının (p<0,0001) hastalıksız sağkalım üzerinde olumsuz etki oluşturduğugözlendi. Ayrıca meme kanseri subtiplerine göre hastalıksız sağkalım süresideğerlendirildiğinde en kısa hastalıksız sağkalım süresi sırasıyla HR(-)HER2(+) ve HR(-)HER2(-) hastalarda saptanırken (medyan 13,9 ay ve 16,2 ay) en uzun hastalıksız sağkalımsüresi HR(+)HER2(-) hastalarda (59,8 ay) saptandı. Hastaların lokal/bölgesel nükssüzsağkalım oranı %92 idi. Kemoterapiye yanıt (p=0,026) ve ER pozitifliği (p=0,020)lokal/bölgesel nükssüz sağkalım üzerinde olumsuz risk faktörü olarak bulundu.Çalışmamızda ; tek değişkenli analizde, nüks varlığı, KT öncesi evre, kemoterapiye yanıt,meme başı tutulumu, multisentrisite ve ER pozitifliği genel ve hastalıksız sağkalım üzerineetkili bulunmuştur. Tek değişkenli analizde diğer parametreler ( genç yaş (≤40), tümörbüyüklüğü, aksilla pozitifliği, kemoterapi rejimi, LVİ ve PR negatifliği) sağkalımı etkileyenbir faktör olarak bulunmamıştır. Çok değişkenli analizde ise, hastalıksız ve genel sağkalımüzerine hastalar arasında istatistiksel olarak anlamlı bir fark bulunamamıştır.
