Abstract:

 

Congenital elevation of the scapula (Sprengel's deformity) is characterized by the higher elevated level of the bone of scapula compared to its normal site. The most significant complication which should be taken into consideration in cases in which surgical treatment must be indicated is the paralysis of plexus brachialis. Scapula was reduced to its normal place by surgical treatment in one of our cases at 5 years of age with a congenital elevation of the scapula. Following surgery a complication of total paralysis of plexus brachialis developed. With the intention of eliminating this complication resulting from extreme tension and compression in plexus brachialis, scapula was displaced more proximally with a second surgical attempt, the arm immobilized in an abduction splint, and immediate electrogymnastic exercises were started. At the end of 3 months the paralysis of plexus brachialis was relieved. Taking opportunity with this case, attemps have been made to review under the light of literature, the paraiysis of plexus brachiaiis resulting from the surgical treatment of congenital elevation of the scapula and choice of its treatment as well.

 

Doğumsal yüksek skapula (Sprengel deformitesi), skapula kemiğinin normal yerinden daha yüksekte duruşu ile karakterize olur. Cerrahi tedavi gereken olgularda göz önünde bulundurulması gereken en önemli komplikasyon pleksus brakiyalis felcidir. Doğumsal yüksek skapula deformitesi olan 5 yaşında bir olgumuzda cerrahi tedavi ile skapula normal yerine indirildi. Ameliyat sonu total pleksus brakiyalis felci komplikasyonu ortaya çıktı. Pleksus brakiyalisin aşırı gerilmesi ve basıya uğraması sonucu ortaya çıkan bu komplikasyonu gidermek amacıyla, ikinci bir cerrahi girişimle  skapula daha proksimale kaydırıldı. Kol abduksiyon ateline alındı ve hemen elektrojimnastiklere başlatıldı. 3 ay sonunda pleksus brakiyalis felci düzeldi. Bu olgu nedeni ile doğumsal yüksek skapula$nın cerrahi tedavisi sonucu ortaya çıkan pleksus brakiyalis felci ve tedavisi konusu yayın;ların ışığında incelendi.
