Abstract:

The number of uncollected culture results for different specimens requested by an Emergency department (ED) in one year were studied and presented with their cost effectiveness, and the approach to manage such increase demand for cultures and its impact on resources. The study included children and adults presenting to ED with different medical conditions. Retrospective review of all uncollected culture results of different specimens requested in the ED between January 2005 to December 2005 and kept in the microbiology laboratory of the hospital was done. The number of specimens received for different cultures was 3922.They included throat swabs, eye swabs, urine , stool , high vaginal swabs, blood, ear swabs, wound swabs and others (vulval, urethral, intrauterine device, gastric aspirate ….) .The number of uncollected results was 1816 ( 47%) and the number of no pathogens isolated (NPI) from uncollected reports was 1120 (62%). The frequently ordered cultures included vaginal swabs, eye swabs, urine and faecal samples and the least ordered cultures were blood and sputum cultures. Implementation of hospital strategy to manage the increase demand of cultures and to follow up of results was done in June 2005 and resulted in improvement in collection of some reports.
In conclusion to manage demand and avoid wastage of resources, ordering of microbiology cultures in the emergency department should be on evidence based medicine, and follow up of the results should be done by implementing network between the laboratory and ED, and by encouraging the patients about the importance to follow up their visits.
